How to Switch to the Choose Life Plate and How to Apply for Funds
Two Ways to Switch to a Choose Life license plate.
1) To switch to a Choose Life license plate ONLINE (no standing in line at the DMV office) and have the new plate delivered to your door, Click Here.
2) How to switch at your county license plate office.
To switch at your county license plate office, take your registration and proof of insurance to the office nearest you. You will pay a one time fee of approximately $40, plus the annual fee of $25, which is tax deductible. Of that $25, the state keeps $5 and $20 is given to Choose Life, Inc. for distribution to life affirming pregnancy centers, maternity homes and non-profit adoption agencies in your county.
After you switch, Email Russ your new Choose Life plate number along with your daytime phone number and address and you will receive a free license plate frame (white with red letters).
